SUMMARY:
Experienced Social Worker with current experience in nursing home and palliative care

POSITION INFO:

 Midrand  Social worker. Elderly, sick and Palliative care. Graduated BA degree in Social work, experienced social worker registered at SACSSP.  Must speak Afrikaans fluently, most of the patients speak Afrikaans and at their age and with their health its crucial to communicate with them in their native language. Must have experience at least 8 plus years, in nursing home, assisted living, frail care. Must have experience to work on psychosocial, mental and emotional level , as well as health issues and consulting with families.  Must have experience in group work. Focus on intake, support services, group work, family meetings and administrative tasks. Must be positive, dedicated, caring, professional , with excellent communication and people skills.  Good computer skills, administrative skills and detail accuracy. Self motivated, passion for the work and a team player. Must stay close. Send detailed CV.
