Our Client, an internationally recognised non-profit organisation, who provide health care services throughout South Africa, to communities at increased risk for HIV and TB, have a vacancy for a Case Officer on a temporary 1 month contract.

A case officer is largely responsible for implementing and monitoring the case management model (CMM) through adherence support for ART patients until they are eligible for decanting to any of the differentiated models of care. The approach uses a tracking system through bulk SMS reminders and then immediate tracing and/or referral if clients miss their appointments. It is envisaged that this approach will improve pre-appointment contact with a healthcare provider so that clients can participate in their care and communicate their adherence challenges. Additionally, it will enable those patients who are at high risk for non-adherence to communicate their challenges beforehand. The target population includes: clients who have been on ART for less than a year including children, pregnant women, mother?baby pairs, unsuppressed viral loads and elicit index client information for follow up.

Main job tasks, duties and responsibilities

  1. Capture patients on case management spreadsheet and allocate dates of return for all care milestones
  2. Establish processes by:

    • Allocating all pre-ART and newly initiated on ART patients according to
      • Minimum 80-150 clients per case officer
      • Geographic information
      • Presence or non-presence of CHWs
  3. . Outline the monitoring system, targets and standards:

    • Phone clients and conduct daily linkage to a CHW or any appropriate community person for those who missed their appointment; visit clients who missed their appointments, patients who are not virally supressed, and patients needing additional care
    • Issue routine reminders according to the schedule of appointments
    • Send SMS reminders
    • Respond to messages from clients and craft an appropriate and professional response
    • Refer patients for enrolment in support groups according to the patient’s preference
    • Prepare for transitioning patients to differentiated care
    • Work as part of a multi-disciplinary team to optimize targets
    • Maintain client confidentiality
    • Revisit index contact testing until all eligible offshoots know their HIV status
    • Provide administrative support in terms of client registers, data capturing and daily reports
    • Maintain high standard of professionalism when interacting with clients and health care professionals

Education and experience

  • Grade 12 certificate
  • At least 2 years’ relevant experience in counselling and testing
  • Adherence counselling
  • Basic HIV testing and counselling
  • Index case trailing
  • Able to communicate in English and isiXhosa is an advantage
  • Enrolled nurse qualification will be an added advantage
