JOB DESCRIPTION:

Main purpose of the job:

  • To work with the facility counsellors, linkage officers and the rest of the project team to provide personalized linkage to care for Paediatric clients with a focus on ensuring initial retention in care

Location:

  • Tshwane (Pretoria)

Key performance areas:

  • Liaise with community and clinic staff to ensure all newly diagnosed HIV and TB Paediatric patients are linked to care
  • Make contact with clients newly diagnosed with HIV and TB to ensure direct linkage to clinicians
  • Provide information to clients and ensure clients have a good understanding of the HIV care pathway
  • Assist paediatric patients and their care caregivers with making appointments for first and subsequent follow-up visit
  • Follow up to ensure clients attend their scheduled appointments
  • Assess each client’s needs and offer support or link them to specialised support as relevant
  • Support clients by answering any queries relating to their diagnosis and the next steps in care
  • Assist clients with making appointments for first or follow-up visits

  • Maintain patient confidentiality at all times
  • Provide health talks on HIV; TB, Infant feeding, PMTCT and Sexual Reproductive Health topics and support patients to access B-Wise and other relevant mHealth sites
  • Participate in health promotion campaigns, including holiday programmes and youth events. These will occasionally occur on the weekend
  • Distribute condoms and IEC material within the facility and community
  • Recruit clients for HIV counselling, TB screening and testing in the community and at the facility
  • Motivate individuals and groups to bring their partners and children for HIV testing and TB screening
  • Take ownership and accountability for tasks and demonstrates effective self-management
  • Follow through to ensure that quality and productivity standards of own work are consistently and accurately maintained
  • Maintain a positive attitude and respond openly to feedback
  • Take ownership of driving own career development by participating in ongoing training and development activities

Required minimum education and training:

  • Grade 12
  • HIV Counselling and Proficiency in Microsoft Office

Required minimum work experience:

  • Experience as a volunteer working in community-related structures and projects for a minimum of 6 months

Desirable additional education, work experience and personal abilities:

  • Good communication skills and must be able to speak more than one African language
  • Experience with NGO-related projects
  • Basic computer literacy and can work independently and as part of a team
  • Be adaptable, approachable, empathetic and willing to take initiative
  • Experience working with people living with HIV
  • Experience in health-related services

Demands of the job:

  • Must be open to communicating messages and implementing educational and social programmes which focus on HIV& AIDS, Infant feeding, Age-Appropriate Disclosure, STI, TB, Condom usage, Family planning and all other issues relating to matters of sexual reproductive health
  • Must be willing to participate in workshops and, when relevant, in the different research projects undertaken by the company

Communications and relationships:

  • Build relationships with the community tracing team and other organizations that we support, including the Department of Health and Ward based outreach teams (WBOTS)
  • Maintain good interpersonal relationships with other partners and stakeholders
  • Work as part of a multi-disciplinary team to ensure patients that are lost to care return to care
