Case Navigators (1 Year Fixed Term Contract) x 3 vacancies are available at our Client, Wits Health Consortium in Soweto, Johannesburg - Gauteng.

POSITION INFO:

Background

Wits Health Consortium (Pty) Limited ("WHC") is a wholly-owned Company of the University of the Witwatersrand, Johannesburg (South Africa) under its Faculty of Health Sciences. WHC provides Faculty with a legal framework within which to operate the research and other activities necessary to support its academic objectives. In addition, WHC offers a range of products and services to the Academics conducting these activities in order to assist with the management thereof

Main purpose of the job

  • To provide guidance to patients on how to navigate the healthcare system by building strong relationships with patients and the relevant healthcare teams

  • Chris Hani Baragwanath Academic Hospital

Key performance areas

  • Provide cancer health education to patients/clients
  • Conduct health screening for clients
  • Establish good relationships with patients, patient care teams and clerical staff
  • Communicate with health care teams to facilitate patient care
  • Identify patient barriers to care and refer to appropriate resources
  • Maintain patient confidentiality
  • Navigate patients within the health care system
  • Follow up with patients to ensure there is no loss to follow up
  • Identify patient’s priority needs and refer appropriately
  • Meet with the palliative care team regularly to discuss patient issues
  • Maintain records of patient interactions
  • Keep track of patient appointments and follow up on missed appointments
  • Prepare monthly reports of a number of patients supported

Required minimum education and training

  • Grade 12
  • Post matric qualification would be an added advantage

Required minimum work experience

  • One year of experience in working with Cancer and Palliative care patients
  • Experience in counseling will be an added advantage

Desirable additional education, work experience, and personal abilities

  • Good cultural understanding of the population served by the hospital
  • Basic computer literacy
  • Good communication skills
  • Basic understanding of the health system
  • Trauma and counseling skills are an advantage
  • Ability to build relationships with different patients and service providers
  • Commitment to good patient care
  • Empathetic, passionate, and trustworthy when working with patients
